Incidence and risk factors of upper gastrointestinal bleeding in pediatric intensive care unit admitted patients

Authors

Abstract:

Background and Objective: To assess the incidence and risk factors of upper gastrointestinal bleeding in children admitted to pediatric intensive care unit for more than 6 hrs. Methods: We prospectively collected medical records of children between 1 month and 18 years old admit-ted to our 10 bed PICU of a tertiary care university hospital between December 1, 2014 and May 30, 2015. De-mographic data, admission diagnosis and basic laboratory data were recorded. We defined upper gastrointestinal bleeding (UGI) as any evidence of blood in nasogastric aspirates, hematemesis and melena witnessed between minimum 6 hours to maximum 5 days after admission. We documented it and detected previously described factors for UGI bleeding such as mechanical ventilation, coagulopathy, organ failure, feeding, and drug prophy-laxis. Finally data were used in a multivariate analysis. Results: Of 157 medical records, 148 patients were eligible. The incidence of UGI bleeding was 34.45%. The most common indication for PICU admission was respiratory system dysfunction (32.2%). Mechanical ven-tilation, thrombocytopenia and prolonged PT and PTT were the significant factors in our study using univariate analysis and mechanical ventilation was the only significant risk factor using multivariate analysis Conclusion: The incidence of UGI bleeding in PICU admitted patients is significant and mechanical ventila-tion is the most important risk factor for GI bleeding.

Effect of Gastric Acid Suppressant Prophylaxis on Incidence of Gastrointestinal Bleeding in Pediatric Intensive Care Unit

Background: Critically ill children admitted to pediatric intensive care unit (PICU) are at increased risk of gastrointestinal bleeding due to stress related mucosal injury. Reducing gastric acid by acid suppressant medication is the accepted prophylaxis treatment, but there is not any definitive guideline for using prophylaxis in PICU patients. The present study aimed to assess the effect of P...

Incidence and risk factors for gastrointestinal bleeding among patients admitted to medical intensive care units.

OBJECTIVES To identify incidence and risk factors for new-onset gastrointestinal bleeding (GIB) in a medical intensive care unit (ICU), a topic for which there is a paucity of recent studies. DESIGN Retrospective cohort study. SETTING Medical ICUs at our tertiary-care hospital, from 2007 to 2013. PATIENTS Patients who developed clinically significant GIB after entering the ICU. Incidence, risk factors and prognosis of nosocomial pneumonia in adult patients admitted in the intensive care unit

Introduction: Hospital acquired pneumonia (HAP) is the second most commonly reported hospital infection and the most common infection in the intensive care unit (ICU). Identification of risk factors and determinants of prognosis in the occurrence of HAP and ways of prevention can be effective in reducing the incidence and mortality of these infections. In this way, we investigated, the incidenc...

Risk Factors and Prognostic Factors of Acute Renal Failure in Patients Admitted to an Intensive Care Unit, Tehran-Iran

Acute renal failure (ARF) is defined as a sudden and continuous decrease of glomerular function associated with azotemia, and may be followed by decreased urinary output. There is a high incidence of ARF in ICU patients with a high mortality rate. Many factors can promote ARF development or influence its outcome. This study was done to assess the incidence, risk factors, outcome and treatme...
